These terms apply to the credentialing services and the text messages of ElevateHP Inc. ("ElevateHP", "we", "us").
The service
ElevateHP helps long-term care facilities credential their practitioners. We collect and check the information and documents of a practitioner for the facility that credentials them.
Text message program
What we send. We send text messages to practitioners about their credentialing file: requests for documents and information, status updates, and reminders of renewals and expiry dates.
Consent. You get texts only after you agree to them, for example with the checkbox on our credentialing form. Consent to texts is not a condition of credentialing or of any service.
Frequency. Message frequency varies with the needs of your file.
Cost. Message and data rates may apply. Ask your mobile carrier about your plan.
How to stop. Reply STOP at any time. We send one message to confirm, and then no more texts. Reply START to subscribe again.
Help. Reply HELP for help, or contact us at the email or phone below.
Carriers. Mobile carriers are not liable for delayed or undelivered messages.
