ElevateHP Inc. ("ElevateHP", "we", "us") helps long-term care facilities credential their practitioners. This policy says what information we collect, how we use it, and your choices.
Information we collect
- Your name and contact details (mobile phone number and email address).
- Your professional details: licenses, certifications, education, work history, hospital affiliations and insurance.
- The documents that you upload, and the messages that you send us by text, email or phone.
How we use your information
- To build and keep your credentialing file.
- To check your details with primary sources, for example state licensing boards.
- To send you requests for documents, status updates and reminders about your file.
- To meet our legal duties and our contracts with the facilities that we serve.
Sharing
We share your file with the facility that credentials you and with the sources that we must check. We use service providers (for example for hosting, email, text messages and phone calls) that process your information only for us and only to provide their service. We do not sell your personal information.
Text messages
We use your mobile phone number only to send texts about your credentialing file.
No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text messaging originator opt-in data and consent will not be shared with any third parties.
You can reply STOP at any time to stop the texts, or HELP for help. Message frequency varies. Message and data rates may apply.
Keeping and protecting your information
We keep your information as long as the facility needs your file and as long as the law requires. We use access controls and encryption to protect it.
